Classroom Reservation Request for Ad-Hoc/One Time Events - Fall/Spring

This form is for the use of "General Purpose Classrooms" during the Fall & Spring Terms only.  Only BSU employees or members of officially recognized student organizations may reserve General Purpose Classrooms.  If you are not affiliated with BSU, please contact Facilities, Operations & Maintenance,  Mark Schimmels at 426-3654 or markschimmels@boisestate.edu.  Note that this is only a reservation request.  The Scheduling Office will send you a confirmation when your request has been approved & processed.  If you are making a change to an existing reservation, you must contact the Scheduling Office directly; bclapp@boisestate.edu.  Do not use this form to make changes.

If you are requesting space for a Student Organization, you must also have your advisor and the Student Activities Office in the Student Union Building sign a requisition to allow use of space, http://finad.boisestate.edu/pplant/facility_req.doc, please fax signed form to Becky @ 6-2351.

You should receive a response to your request within 2 business days.
