Academic Adjustment
This form is used to waive a graduation requirement (may require an appeal) or substitute a transferred course for a graduation requirement.
Academic Adjustment for Transfer Credit
This form is used to create a rule for transfer equivalencies.
Academic Appeals
This appeal form, which is submitted to the University Academic Appeals Committee, is used for adding & dropping classes after the deadlines, past semester complete withdrawals, early reinstatements, most academic adjustments, and changing from audit-to-credit or credit-to audit after the deadlines.
Catalog Year Update
This form is used to change a student's catalog year used for graduation requirements.
Credit for Prior Learning Form
This form is used to obtain academic credit when a student challenges a class, has passed a higher level class and wants the prerequisite(s) for that class, or has a portfolio showing competency in a class.
Diploma Replacement
This form is used to order a replacement diploma for a degree or certificate previously earned at Boise State. Payment must accompany the form.
Directed Research Application
This form, with the specific approvals, is required when a student wants to be enrolled in a Directed Research course.
Early Reinstatement Plan of Study
This form must accompany an Academic Appeal form for students requesting early reinstatement after a dismissal.
Exceed 21 Credit Hours Request
This form is required for any student intending to enroll in more than 21 credits. The student must gain advisor approval to enroll in over 21 credits.
Grade Exclusion
This form is used to remove a term or terms from the GPA when the student meets the specific requirements.
Graduation Application Update Request
If you have already submitted a graduation application, and need to update your graduation term, catalog year, major, minor, or certificate, use this form to update your graduation application.
Idaho Residency Determination Worksheet
This form is used by students who are trying to establish residency for tuition and fee purposes and must accompany the required documentation listed.
Independent Study—Graduate
This form, with the specific approvals, is required when a graduate student wants to be enrolled in a Graduate Independent Study.
Independent Study—Undergraduate
This form, with the specific approvals, is required when an undergraduate student wants to be enrolled in an Independent Study.
Information Update
This form is used when students would like to update their biographical information including their Primary Name, Date of Birth, Social Security Number or Academic Advisor.
Permit for Seniors to Take Graduate Courses
This form is required when an undergraduate student enrolls in a graduate-level course.
Reading and Conference Application
This form, with the specific approvals, is required when a student wants to be enrolled in a Reading and Conference course.
Receiving Credit for English Composition Based on Test Scores
With this form, students can receive test credit for ENGL 101 and/or ENGL 102 based on satisfactory ACT/SAT scores.
Receiving Credit for English Composition (Initial Enrollment Prior to Fall 2013)
With this form, a student who had enrollment prior to Fall 2013 can receive test credit for ENGL 101 and/or ENGL 102 based on ACT/SAT/Compass results.
Registration Override
This form is used when students would like to add or drop courses they are unable to do on their own due to system restrictions. Late adds must accompany an Academic Appeal Form.
Release of Information
This form must be filled out when students would like to give the university permission to give out information protected by FERPA law.
Request to Exceed Maximum Course Registration
This form is used when students are attempting to retake a course after the second attempt, requires an academic advisor and department chair signature.
Request to Repeat a Graduate Course
To ask to repeat a graduate course, a student submits this form to the Registrar's Office after approval from the program coordinator.
Requesting Approval to Drop a Course After the Deadline
This form is used when a student requests to drop a course after the last day to receive a W for each session. For session deadlines, see the Academic Calendar.
Second Baccalaureate Degree Transfer Credit Request
This form is used for second degree seeking students who work directly with the department of their degree to use prior courses as prerequisites for their current program.
Senior Citizen Fee Rate Waiver
This form is used when a student opts out of using the Senior Citizen Fee (reduced fee) in order to gain a degree-seeking continuing student enrollment appointment.
Student-Athlete Drop
This form is used by student athletes who need to drop a course during the current semester.

UF Waiver—Boise State Core Certified
This form is used to help students who attended Boise State prior to 2012 determine their university foundations placement
UF 200 Course Placement
This form is used to help transfer students determine their university foundations placement.
Withdrawal/Re-Registration Affidavit
Students who receive federal financial aid are required to complete this form when they have dropped all of their courses but decide they would like to re-enroll in courses for the same semester.
